This symposium focused on how high quality demographic data can be used to drive equitable care from system to organizational to clinical levels, while also ensuring that data is being collected in a way that is inclusive of and accessible to all our patients.

This workshop focused on the demographic data collection efforts of the Toronto Central LHIN Community Health Centres (CHCs). The TC LHIN CHCs came together to share strategies, tips, and tools for collecting and analyzing client demographic data and to help identify where CHCs need further support.
