About this guide
This guide is based on the work of the Tri Hospital1 + Toronto Public Health pilot project and Mount Sinai Hospital’s own experience developing a demographic data collection plan. The information provided also stems from an extensive literature review, a scan of global best practices, consultations with leading practitioners and researchers, and interviews conducted with hospitals in Canada, the United States, and the United Kingdom.
We want to thank the Toronto Tri-Hospital+ Collaborative (St Michael’s Hospital, Centre for Addiction and Mental Health, and Toronto Public Health) for their hard work and commitment to sharing the learning and resources with other health-care organizations. The Human Rights & Health Equity Office would also like to thank the Toronto Central LHIN for providing ongoing support for this project. Together, we have increased health equity within our organizations.
The work on this guide is funded in part by the
Toronto Central Local Health Integration Network (TC LHIN).
