Mount Sinai Hospital, St. Michael’s Hospital, the Centre for Addictions and Mental Health, and Toronto Public Health known as “The Collaborative.”1 completed a pilot project aimed at developing and testing a survey tool.
The hospitals developed this tool using 13 items; following pilot testing in multiple sites, the collaborative derived a set of 8 core questions and 3 optional items, the latter of which could be adopted by hospitals based on their needs and demographic data collection goals.
